Anne A. Diaz was born on July 24, 1930 in Gary, Indiana to Thomas and Jaza Pusak (Slavich) and passed away on November 28, 2014 in Loma Linda, California. Anne was a resident of San Bernardino County for 12 years. Anne is survived by her loving children Sebastian and Jaza Diaz.
